Sick Gourami

I hope someone can help me my poor little sweetie (blue gourami) is sick. I hope this information will help.

1. Water parameters. all within safe or ideal

2. A full description of the fishes symptoms. The fish looks fine and acts fine most of the time. then all of a sudden it starts to flip out, swims all around jumps out of the water, bangs on the glass, Like it is having a seizure. Then it finally relaxes it can't seem to get its bearings right away and swims upside down and sideways and has trouble going to the top for air. While it is like that its eyes pop out at a wired angle. and then it finally uprights itself and acts normal except that she goes up to the surface for air.
The attacks have been coming more frequently but she recovers quicker each time. I don't know what to do none of the other fish appear stressed in any way.

3. How often you do water changes and how much. Every month 20-25%. Two months ago the dechlorinator I used must have gone bad the water was cloudy and the levels spiked. Some of the fish were stresses but the problem was fixed and everyone had been fine for a month and a half.

4. Any chemicals and treatments you add to the water. Declorinator, salt

5. What tank mates are in the tank. Pleco, cardinal tetras, tiger barbs, shark, danios, molly
6. Tank size. 55 gallon
7. Finally Have you recently added any new fish? About 2 and a half months ago several died within weeks from something an angelfish had but everyone has been fine for a month or more
 
hiya, sorry 2 hear that ur fish is sick, seems like u have done everything that u can 2 get help! so well done 4 that as it will help ppl know how to help u.

rite, have u had a really gud close up look at the fish herself ? the main areas id suggest u 2 check are her tummy, face and fins. if these all seem normal then ok, im not sure wot else to check. but if not, let us know.

have u tested ur nitate levels ?

I have noticed that the gourami's face has a pinkish hue but nothing else appears to be different.
Also, I remembered that about a month ago she started banging her head on everything: the heater, the thermometer and the tubes for the filter. At first I thought she was just upset because we had new fish, but now I am wondering if it is possibly nureological.
