Royal Gramma Inactice With White Spot

My royal gramma is inactive, not eating, losing color and has a white spot near the top of the gill cover. The spot is bigger that the spots you would see for fresh water ick.
It stared with it scratching in the sand. I originaly thought it might be flukes since no spots were visible. I treated with prozipro and the scratching stoped but came back again in a week.
