Refresh Substrate?

I'm wondering if over time if you need to do anything to your substrate to reinvigorate it. I have a moderately planted tank and originally put down some laterite. Do I need to do anything to the substrate to make it more hospitible to plants now that it's been up and running for a while? I read somewhere that the substrate can get "stale". What exactly does that mean?
 
Substrates can get used up or got 'stale' over time but I reckon you'd have to work at it i.e. lots of plants, high light, fast growth. Don't forget also that fish waste will renew the nutrients in the substrate to a certain extent. It also depends how much nutrient there is in the water, less and the plants will have to rely on the substrate for their food.
