Puffer gill problem

I have a figure 8 puffer who is normally seems fine but sometimes has problems with one or the other gill looking closed or partially closed. He usually becomes less active when this happens.

Amonia and nitrates are at 0, ph is around 7.8, salinity is around 1.008. I had some coppersafe in there to kill off some wormy looking things. Most of that's gone now. Anyone know what this might be.
 
How long does the gill stay closed? Puffers do open and close their gills to breathe. It may still be something to do with those worms. Where were those worms?
