Post Mortem?

Not an emergency, it's too late, but I'd be interested in diagnoses.
A few months ago this hengelis rasbora had a white spot so he went into the Q tank treated with anti fungal then anti bac meds. Otherwise ok, eating etc. After a few weeks, he seemed fine, still had the one spot.
Put him back, been fine since. Last night noticed some slight white filmy patches so out to the Q tank again, dosed but he'd gone tonight.
See how his upper jaw has receded, you could just see that starting. And his eyes are bulgy.

the bulging eyes could be a sign of pop eye which is caused by a bacterial infection I believe
 
Can pop eye cause the upper jaw to recede though?  I can't see any gill or fin issues.
 
I don't know much about pop eye - only dealt with it once and the fish that had it had Hexamita too (pop eye and hole in the head can develop because of Hexamita) so she didn't make it.
 
Research into pop eye might hold the answer to the jaw problem too
 
Couldn't it be an injury that caused the jaw to recede and in turn the bacterial infection?
 
Thanks everyone :) I'm giving the Q tank a bleach out and then get some more fish for main tank next weekend if the rest look ok. Fingers crossed....
 
Maybe his immune system was affected from the previous infection making him more susceptible to other illness
