The lump on head is not good at all you need to get a hospital tank seperate from your big tanka 10 gal would be best just put something in it to hide in and that's it.
I had a ram develop that pump on the head too. It ended up splitting open it was awful. I used Maracyn 1 and then 2. It helped because it is a bacterial infection. If your fish is getting bullied at all this will not help him heal and that's way a hospital tank is much needed.
Set up a hospital tank try some maracyn 1 then if that did not help try maracyn 2 . There it no sure way to tell if it is a gram positive infection of gram negative. Maracyn 1 treats the positive and 2 the negative.
Good luck and remember clean water is the best way to help a fish heal. When I treat a fish I do the treatment then once done I do large water changes everyday after. In the hospital tank.
