Poorly discus

Hi there

I have a very poorly discus - he's recently developed 'holes' / ulcers around his head, his eyes seem a bit swollen too. I gave him a dose of Interpet No 9 - Anti Internal Bacteria, as I had some to hand at the time. This didn't really seem to help, so after a water change switched to No 8 - Anti Fungus and Finrot, this was on Saturday. Since then he seemed to be on the mend, much more lively taking food again, seemed fine when I fed him this morning...

This evening however he's on his side near the bottom, seems to be stuggling badly - what can I do?

All mg/l (except pH, obviously...)
 
More tank details:

36x12x15
10% water change every two weeks, test results stable
Only treatments used are those mentioned above (other than water conditioner when changing)
Tankmates - 7 platies, 1 siamese flying fox, 1 plec, approx 15 cardinal tetras, no recent additions
