Tank size: 5 gallon
pH: I'm so sorry I don't know the readings to any of these. I don't have a test kit but the last time we had the water tested they told us all the levels were great. I'm sorry that's all I can tel l you.
ammonia:
nitrite:
nitrate:
kH:
gH:
tank temp: I just looked and it's about 79 which is a bit high.

Fish Symptoms (include full description including lesion, color, location, fish behavior): I noticed my pink glofish not eating about 4 days ago, and has become more slender. It ate a bit yesterday. It was staying in one spot near the heater for the most part of the last 2 days. It didn't leave that General area except for when I feed them. "tetra color tropical flakes" No visible lesions or spots. We went to the store and came back and it was stuck on the filter so we turned the filter off and it started to try to swim but it couldn't go anywhere and it floated to the top and we took a jar and put some of the tank water into and put the fish into it and now forbthe last hour nor so it has been floating around in there breathing and occasionally trying to swim but it can't. It's on it's side right now.

Volume and Frequency of water changes: once a month 25%

Chemical Additives or Media in your tank: I know my parents added some chemical to the tank, I have no idea what it was. But this was a good month ago. Not recent. P

Tank inhabitants: 1 albino cat fish? I think:/ 2 neon tetras 2 glofish and an algea ball. All under an inch long.

Recent additions to your tank (living or decoration): none

Exposure to chemicals: I'm not sure

Digital photo (include if possible): sorry n/a

dont know what is wrong but you need to get a bigger tank and you need to get 4 more glofish and 4 mor neon tetras because your groups are low they are schooling fish

First off, you should do weekly water changes, not just monthly...I have a ten gallon and my water change routine is a 10%-15% weekly and 30%-40% monthly...

And 79 degrees should be fine :) just keep an eye on it if it is warm where you live...it's 125 degrees, give or take a few, where I live...i have to add ice cubes to keep it under 83 or 84 degrees!

And I don't know MUCH about catfish, but I'm pretty sure that they should be kept in large groups..like 6+, which, unfortunately, you don't have room for all of those in your tank :/ Maybe someone who knows more about them can comment on this :)

And my suggestion is to get a new small tank and cycle it for a quarantine tank...it's not good to put a fish in a jar, even if it is sick...You don't have to make your quarantine tank as decorative as your display tank...it's just nice to have an extra cycled tank on hand when something goes wrong :)

Yes you need to be doing that change weekly not monthly. I suspect that has caused problem. Do a big clean and water change at least 50% and get yourself a test kit and test amonia and nitrite right away.

I agree with everything that has been said and if you have the following image as your white catfish

It's an albino cory, and yes the should be kept in groups of 6 or more.
