She has a goldfish that will float at the top of the tank upside down. It does this on a regular basis but seems to do everything else like a normal goldfish.

Something will be wrong with it's swimbladder, which is basically what ballasts the fish in the water.

It's very common on goldfish since they rarely have adequate tanks, the resulting bad water conditions lower their immune system leading to the swimbladder infection.
'Fancy' twin tailed goldfish like orandas are even more prone to it.

Have you got any information on tank size, filtration and maintainance schedule?

If the fish is otherwise fine the best thing to do is get the tank in top shape, then use something for internal bacterial infections, I'd recommend pimafix, I've seen it work very well in the past and it doesn't seem to kill useful filter bacteria like a lot of other meds do.
