Medicating a tank w/a filter

So I haven't ever had to do it, and I don't need to, but was just wondering.
I've heard 2 diff things about medicating when you have a carbon filter.
I've firstly heard that you need to cut all the carbon out when you're medicating, and then I've also heard that you don't need to cut the carbon out when you're medicating... as long as it's a fully cycled tank.

Can someone straighten that out for me?
 
If they are black carbon sponges in the filter, yes you should remove them for medicating as they will just remove the meds from the water.
 
I have filter "bags" that hold loose carbon inside. I always removed the carbon before medicating a tank as it will remove the meds. I stopped using carbon all together now because I don't want any chemicals to leach back into the water. Also, if I do have to medicate (like right now cause my poor angel has velvet) I don't have to remember to remove the carbon. When I am done medicating, I usually float a sachet of carbon in my filter to remove remaining medication. I then take it out and discard the carbon after about 3 days.
