Holes And Torn Leaves

Hello. I have 1 panted tank for my betta fish. It's a 5 gallon tank with 1 amazon sword, 2 gold ribbons?, 1 small Anubis, and 1 small moss ball. It's not by the window so I have to use artificial light. I have a blue bulb in the hood. All of the plants look fine except for my amazon sword. It has holes in 1 leaf and 1 leaf is torn off at the top. I use API Leaf Zone as fertilizer. What happened to my sword? Also, what type of light bulb should I have?

i dont think there is anything wrong with the leaves (i may be wrong) but i think you should take the leaves out just to be sure.
 
Just take out any broken or torn leaves as they won't repair themselves, get them off as low as possible to the substrate and keep an eye on the plant as blue lights aren't normally very plant friendly, if your sword is new in your tank this will happen to a few of the leaves as they get used to their new surroundings.
 
The sword has been in the tank for a few months. What kind of light should I get?
