Hole in head disease?

Is this hole in head disease please help
its a 5 gal planted tank with her and a few snails
i just found out hitched a ride on a plants

do a water change and add 1 heaped tablespoon of rock salt for every 20 litres of water. keep salt in tank for 2 weeks,

There is also that white thing on her head where she lost a scale idk what that is

There is swelling and excess mucous around the left eye but the fish just looks unwell. Clean water and a bit of salt and see how it goes.

That mucous had been there for at least a year what fo you think it is, her vision has gone down alot, she is about 3.5 years old now

Should i do it in a quarentine tank? I do not want to kill the plants. The quarentine tank only keeps like 2 gals of water but is 3.5 i have one decoration and a plant in there or i could just treat with salt in

I do water changes but my water never seems to get ger clean, I have a filter meant for a 19 gallon and wash it out in aquariam watwr fequently but i cant get debris out of my tank, the paraneters are not the best right now because some of my plants are dyinn so i think i need to take those out
