Gourami with a sore 2

One of my two dwarf male gouramies has a sore near his mouth and now I can see something white sticking out of it, no more than 1.5 millimeter long, looks like it's some sort of a parasite. What kind? All the advise I can find in the books and online, says that visible parasites should be removed manually with the tweezers. But I have no idea, how to do that. How should I handle him to keep him still? In the water or not? Help!
