Gold Severum Needs Help !

Hi, can anyone help by telling me what this is on his face, water parameters spot on using api master test kit, 25% water change 2 days ago . I have a parrot in the tank with firemouth cichlid, clown loach and common plec.
Tank is 210ltrs running at 82c (28f) and has been for the 4 years it has been setup, Eheim 2217 external filter. Never had this problem before and i'm wondering if its been attacked, any help would be much appreciated.View attachment 67343
