Geo Tapajos with cloudy pupils in eye

I have a geo that is about 1.5 years old that has a cloudy pupil in eye. I have him in my hospital tank for observation and have tried the aquarium co op quarantine trio on him. It does not look like a cloudy eye from the pictures I see on the internet. Does anyone have any idea? Salt also does not help him.
  • pH 7.0 to 7.2
  • Nitrates 25 max
  • Hardness Low
  • Nitrite 0
  • Ammonia - 0
  • KH/Buffer - Low
  • Water Temperature -76 to 78 controlled by a thermostat.

None of these readings provide the most important info for dealing with growing Geophagus.

How often do you do water changes, and at what percentage?
 
As my Tapajos grew, I had to do 40% weekly, then bi-weekly. I gave them to a friend with a much larger tank. I was using a 75 gallon. I switched to a 120, and then realized they were becoming a job. If I missed a water change, I saw cloudy eyes, which cleared easily in clean water.

They do very poorly with salt, btw, as fish from soft rainforest water.
