Fungus Not Going

I am getting very worried, as am not able to get rid of fungus in the tank. It started 4 weeks ago, and has been getting better and worse in 'cycles'. Sometimes it affects the eyes, sometimes fins, back or head.I have used salty chloramine water and malachite green. All the water readings are fine (ammonia 0, nitrite 0, nitrate .25, ph 7,5. What could this be? Is there some other possibility than fungus? It started after I got a new fish, which was in quarantine only for a week, but he is not ill!! The thing definately looks like fungus, its sort of cotton wool like white fluffy thing. I have separated the ill fish.
What else can I do to easy their fungus and get them to be happy and healthy again?

You need a bacterial med for columnaris what your location.
 
Many thanks for your replies. I am getting a different medicine today then, from my lfs. getting really worried, as it has been going on for so long. Should I clean the whole tank and everything in it with hot water and bleach? I would have to put the fish in a bucket, tho, and also I know by doing that my tank would be uncycled again. But by what I have read, this may be the only thing to get rid of columnaris. The fish would then have to be in uncycled tank, and I could change 30% every day. Would that be ok? Any ideas?

I agree with Wilder. I would start antibiotic therapy and slowly increase tank temp after the antibiotic is added. Would also add aquarium salt. Don't forget to remove your carbon from the filter. SH
