Fluval Fx5 Media Setup

Hi All,
 
Just recently purchased a 7 month old Fluval FX5 canister filter for my 350L to 400L fish tank and I don't really know what media has been replaced and what hasn't so I was looking at buying some new media for it and need your recommendations. At the moment its running:
 
Top Basket: 1 Fluval Fine Filter Pad which looks a bit ratty, and Plastic Bio-Balls I think they are.
Middle Basket: 1 Fluval Carbon Bag, and either a Fluval Zeo-Carb or Fluval Ammonia Remover Bag, I can't really work out what it is. 
Bottom Basket: 1 Fluval Fine Filter Pad which looks a bit ratty, and either Fluval Pre-Filter or Fluval BioMax, once again I'm not really sure which one it is
 
What would you recommend I replace the media with and how would you recommend I set it up? I don't know how old the media is which doesn't help.

I think owners of these filters use all sorts of combos, but I keep mine simple, Eheim Substrat Pro in each basket and the standards sponge pads that wrap around each basket.
