Fish turning black /blind

Hello. I recently noticed that my schooling bannerfish started to turn black in the front of his face. I also suspect that he’s going blind, because his eyes look black/cloudy, he doesn’t see food, and sometimes bumps into fish. He also doesn’t run away from me when I come near the fishtank, as he usually did before. He was also always the first one to eat the food when it would drop, now he’s lucky to even get one piece by accident. Please advise me on what I can do, he is my favorite fish and I would absolutely hate to lose him. My fish tank is around one year old, I do water changes Once every two weeks, 15%.
