Couple Of Questions

The turtle (now known as Crush) that I rescued the other day is doing great! He eats from my fingers and is just so awesome to watch.
I have noticed that he has a few soft spots on his shell, as well as a few places along the edges where the shell is slightly split. What can/should I do about this?
So far I have a UVB light that sits above his basking area, and I have the calcium/vit D supplement that I roll his food around in 3-4 times a week. Is there anything I'm doing wrong or missing?

Also, he seems to swim and just peaks his head out, not really ever basking. Is this normal, or is bc I don't have a heat lamp yet?
