Clown Loach Acting Strangely

I think my clown loach is sick.
A week ago I found him inside a cave in my tank upside down. I thought he was dead. I moved the cave and he swam out all normally.
Today I say hm sitting sideways on top of the filter. I moved the filter and he just swam to the bottom.
He has been acting like this for the last 2 weeks now. He also seems to have much skinnier and doesn't seem to be eating.

All the other fish in the tank seem to be perfectly healthy. Any idea what the problem may be and how I can help him?

Being upside down or on its side is perfectly normal, albeit it gets unprepared loach owners worried.

Not eating immediately may be normal; 3"+ loaches may not be interested in every meal. Offer something tasty, like frozen bloodworms.

Getting skinnier is certainly not normal. Usual cause: parasites. All loaches should be dewormed. Post a photo to make sure.

He is probably just acting the clown
Do you have some sort of cave in the tank?
What size is the tank?
