Adf With White Eyes

Hi all, I've recently put my ADF into a species only tank and given her some friends (2). They have now been in the tank (28l) for a few weeks and whilst the two youngsters are doing well, the original frog isn't. Her eyes have gone white, first one then the other and she spends most of her time floating. I haven't seen her eat for ages, even when waving bloodworm in front of her nose.
The tank has a sand substrate, a few live plants (Java fern, moss ball caves) and the filter is on its' lowest flow setting with media taken from an established filter. I am also doing a 25%-50% water change per week. The only other occupants are 3 female cherry shrimp.
If it was just the eyes I would think she was getting ready to shed but she's also behaving oddly and not eating. There is no other external signs of disease etc.

Check your ammonia levels first and report back. White eyes can be a sign of poor water quality.
 
Thanks for replying. Unfortunately she passed away earlier this afternoon :(. I tested the water anyway and the ammonia was 0 (or at least that's what my eyes told me, there's a possibility that it was ever so slightly coloured but I couldn't tell). The filter was filled with media from an established filter from another tank which had been running for several years. Her eyes are quite hard to desribe; they weren't cloudy, more like someone had drawn white circles around the edges but left the middles black?! The other two are fine with no symptoms at all.
RIP litle froggy :(
