pirahnas are deaying on me

Already 3 of my piranhas have dead on me plz help. I have had my piranhas for over 6 years. They are in a 225gl tank. Symptoms are lazy behaver and really sluggish. But my main consern is there eyes, there murky and the ones that died went blind and there eyes like caved in. I have never had any...
