Holes likely occur when bettas catch their fins on sharp decor whether it's ornaments, plastic plants or hardscape. Remove anything sharp and change the water frequently to heal the tears

What filter are you using? Chances are it has carbon in it which is leaching into the tank. Best thing would be to throw away the carbon and replace with sponge
