Your tank should be cycled by now. Do as Colin_T instructs. I don’t think you need to begin completely over again. Perhaps get some Tetra Safe Start + and add to the filter. Don’t use Prime for 24 hours after you do this. You can then start using Prime again to protect your fish after that...

OP’s readings are within range so tank is cycled. Does appear to have a tad bit of nitrite though. Can you check your tap after to see what it s?

Normal dose is 1 tablespoon per each 5 gallons of tank water. So use 1/2 tablespoon.

As this a deformity, is the fish suffering. He seems to lose control in the video and gets knocked over. Would euthanasia be better?

Salt can help with same external parasites but not internal ones. Something like Prazipri will treat internal ones. I would treat in the tank too be sure it is treated too. :)

Not regarding stocking, but why don’t you add some of your media from your established 5G to speed things along? That or some of the substrate.

Yes, you can use them together. You need the ammonia to start the cycle. All the Safe Start is doing is adding some good bacteria to move things along. I use to use both before I had additional media in my stable tanks. :)
