The fish is a week old. I added a honey gourami 3 days ago but they get along just fine. I did get rid of a betta that was bullying the ram, so I replaced him with the gourami. The tank has been going 4 years. I have not noticed him scratching. I just did water a change last Saturday with a filter change. How long has the tank been set up for?
Is the fish rubbing on any objects in the tank?
Check the water quality for ammonia, nitrite, nitrate and pH.

If you need to treat white spot, just do a 80-90% water change and gravel clean the substrate, then raise the water temperature to 30C (86F) and keep it there for 2 weeks, or at least 1 week after all the white spots have fallen off the fish.
Clean the filter too if it hasn't been done in the last 2 weeks.
Make sure any new water is free of chlorine/ chloramine before it's added to the tank.

I changed out the activated cabon filter pad but there is a biological filter inside the reservoir so im not losing the beneficial bacteria. I only clean the water pump when it get cloggedCould be white spot but blurry pics make it hard to tell.
Treat it as if the fish has white spot and raise the water temp to 30C and keep it there for a couple of weeks. The temp won't hurt the fish but should kill any white spot parasites.
When you say changed the filter last week, do you replace the old filter media with new stuff?
If yes, then you could have an ammonia and or nitrite problem due to getting rid of the old filter media.
Power filters should be cleaned at least once a month. However, if the filter is less than 6 weeks old, do not clean it. When you do clean the filter, wash the filter materials/ media in a bucket of tank water and re-use them. Tip the bucket of dirty water on the garden/ lawn.
