Looks like amazon sword, best to take it out if the pot, take the foam wrap off & plant it in the substrate
 
yup its an amazon sword (Echinodorus sp.) you really need to plant this in the substrate because they are heavy rooters

make sute that the roots are buried well or it will float.
 
I kinda hooked a few roots around the sinking rock disk that came in the pot, should stay down.
 
yep lovely looking plant, i love these one of my fav, but you see that brownish patch (crown) on the bottom than needs to be level on just under the gravel, so all the roots are under, if you have not had it long expect it to melt back, but it will grow some new leafs soon, just bare in mind they may look completely different to the leafs u see now as well depending on how this was grown before you acquired it 
 
this was mine when i sold it :)

Will it stay as one clump or send out shoots to make new plants, I know my java fern reproduced like crazy and I went from one to 6 plants, will this also happen with the sword?

It depends on how many subplants there are, Mine had 3 separate plants
 
the one you see on my profile pic is one of my 3 plants
