Whats Wrong With Its Eye?

the left eye obviously....i know its a bad pic, but i just wanted to show how big it is compared to the other eye.
 
You're sure you mean nitrate? Nitrates can be high and still not cause too much of a problem. We usually say under 40 or so.
It's NitrIte you need to be careful of. Ammonia aswell.
 
nitrite and ammonia are at zero, thanks....

nitrates are spiking between 20-50ppm
 
but i have had the tank running for a while now, and no problems....till now. so i dont see how it could be the tap water.

Well, how often are you cleaning the tank? I don't think it's out of the ordinary for water parameters to fluctuate in tap water, especially something like that. I would check the tap, that would be my guess.
