When I got my gourami a few days ago I did notice something about his gills but thought it was normal as I have never had one before. Then tonight I looked at the other side and realized one side is normal and one side is missing like half of his gill. Should I return him, Is he going to die?

It can be genetic.
As long as you maintain excellent water quality the fish should be fine, as it can soon get infected.

Can you tell whether it is naturaly like that or the result of an injury? No way to tell from the pic. Like Wilder says, he can survive like that but he'll need clean water. Also, BTW, are you sure he was like this when you got him? If there's a fish in your tank that may be responsible, it's best you work out which one it is before it causes even more serious damage.
 
Well I would think it was the angels because they didn't get along so hot at first. But when I got him home and was in the bag I noticed it and he was in the bag alone. at the fish store he was in a tank with other gouramis. I can't really tell if its a wound or not but no redness swellin or anything just looks like a hole.
 
He's not laboured breathing is he.
It can be born that way.
Or bacterial, fungal, to parasite.
 
He does seem to be breathing pretty fast but is eating and swimming fine. He even made friends with the angels. Do you think I should throw in some pima fix just to be safe?
 
Don't like the sound of him breathing fast.

For now just add some melafix.
Any signs that he rubs his gill on things.
